Registration Dates for the 2024-25 school year are as follows:

Fall

 May 13th to July 12th 2024

Winter

August 23rd to October 25th 2024

Spring

December 6th to February 7th 2024

 

Please check with the nurse's office to make sure that you have a valid physical on file!

 

IMPORTANT INFO, PLEASE READ BELOW:

Athletic Pre-Participation Physical Exam Forms are ONLY being accepted electronically. Once your doctor visit is complete please upload the form to the database in your family account.

Physical Form

To participate on a school-sponsored interscholastic athletic team or squad, each student shall provide a health history update questionnaire completed and signed by the student’s parent or guardian and will need to be uploaded under your family account for each season.

Impact Concussion test is required by all students starting their 9th grade year.  Test is valid for 2 years and will need to be completed again during their 11th grade year .  Please click the information below to access the instructions to complete the Impact test.
